Fix sphere and LSS tests under D3D12 and Vulkan (#476)

* Fix incorrect geom type when converting LSS inputs

D3D12 and Vulkan incorrectly set the geometry type to spheres when
converting LSS build inputs.

* Enable sphere/LSS geometry during VK pipeline creation

* forgot file

* enable LSS & sphere support in D3D12 pipelines

* Account for struct alignment differences in LSS & sphere tests

* Only supply builtin intersector under CUDA

* Disable failing hit object tests

* add triangle attributes to CH programs

* Enable LSS vulkan features

Without enabling these features, validation layers will complain.

* Fix vulkan pipeline flags handling

* Add comment on global state
