Align @skip and @include directive checks on fragments with spec (#3710)

* move shouldInclude check to happen before de-dupe

Update CollectFields to more closely follow the reference implementation
found at https://github.com/graphql/graphql-js/blob/16.x.x/src/execution/collectFields.ts

* add reference tests for skip and include from graphql-js

* lint
