parse: Fix bug where only 2 or fewer causes/effects could be added

Fix a bug in the parsing logic where only two causes/effects could be
added.  The logic to add causes/effects to the end of the linked lists
in the rule didn't walk to the end of the linked list before adding the
cause/effect.
