feat: policy filtering now includes reason

BREAKING CHANGE:

The response object from a `.filter` now includes all the vulns (whereas before they were stripped), but each filtered vuln includes a new object property `.filtered` with the `type` and additional `metadata` object (where applicable).

This doesn't change the API, but does change the expected objects.
