[FEATURE] Add `Create` command

The command inserts a single row into the column family.
Modifiers `#insert` and `#using` are available.

    class CreateName < ROM::Commands::Create[:cassandra]
      relation :items
      register_as :create

      def execture(id, name)
        super insert(id: id, name: name).using(consistency: :quorum)
      end
    end

    create = rom.command(:users).create
    create.call(1, "Andrew")

Notice the `Dataset#insert` prepares data for the new row, while `Dataset#create`
executes the CQL query.
