Changes:
* old-ie-hacks.js removed, new code for IE8-9 is inside a IIFE in lib/browser/tag/mkdom.js, and reduced to one small function with support for tables with looped cols and multiples bodies.
* The function for create specific IE8-9 elements is not changing boolean attribute names anymore. This is consistent with others elements created by handmade riot.tag's, the compiler do this work and users must add the '`__`' as for any other element created by riot.tag. I see no issues with 'label' property in optgroup, even with expressions without the 'riot-' prefix.

Reordered wrap/prefix.js:
* isArray does not check `Array.isArray`, they must come from a polyfill, needed for IE8 users anyway.
* Renamed ieVersion to IE_VERSION, for consistence with other const/vars (I assumed is for internal use).
* New constants RIOT_PREFIX and RIOT_TAG
(I have adjusted the code to not depend on literal 'riot-' values, except some regexp)

Fixed #1067 - there's no visible issues with looped cols inside a colgroup.
Fixed #815 and minor adjusts to update.js/update().

Added generic function `startsWith()` to tags/util.js, mainly for using with update(). It is faster than native ES6 `String.startsWith`.

Added test for `<select>` tag with `option` + `optgroup` with unclosed options (HTML5 specs allow this), for tables with multiple bodies and dynamic styles, and for tables with caption and looped COLs, TRs and TDs.

**NOTE:** removed the exclusion of Istanbul for testing IE9 specific code in mkdom.
