PERF: additional speedups to `nj` (related to #431) (#1513)

* refactored _lowest_index and _compute_collapsed_dm

* updated CHANGELOG.md with PR number

* changes from @wasade & vectorization of joined distance calculations

* removed _otu_to_new_node & tests; removed unnecessary calls to to_data_frame

* actually removed _otu_to_new_node

* switch from an array conversion to concatenation

* removed a commented-out line

* added test notes to CHANGELOG
