Make IIIF Image API compatible with S3 backend storage, re #204

Make the new storage approach for saving generated images compatible
with an S3 backend, or remote storage backends in general.

- redirect to remote storage engine URL when available instead of
  piping image data through the local server
- improve cache-busting approach to embed Image modified timestamp info
  in file storage path, rather than doing fragile time comparisons
- refactor storage path creation code into utils module.
