fix: ignore password, remember_token, and api_token in history by default (#100)

Introduce a config-driven default_ignored list applied at the filter
chokepoint in getHistoryChanges(), so sensitive fields are never
persisted in the changes column even if the consumer forgets to call
addIgnored(). The model's own $ignored list is still merged in and takes
precedence; consumers can override default_ignored in their published
config.
