Fix review approval gate writer (#1969)

* fix: allow reviewer to open QA gate

Permit the review node's reviewer agent to write the review approval gate so approved PRs can advance to QA without requiring the autonomy fallback.

* fix: restamp workflow gate writers

Include gate writer changes in built-in workflow hashes and re-stamp matching gate fields so existing spaces receive the review approval gate fix.

* test: sync review gate fixture writer

Keep the web workflow fixture aligned with the canonical QA review approval gate writer.
