refactor(mcp): convert mcpRouter and mcpServer to TypeScript

Add @types/koa and @types/koa-router as devDependencies. Rename
mcpRouter.js → mcpRouter.ts and mcpServer.js → mcpServer.ts, adding:
- McpState interface with JSDoc on each field (r/cc/os)
- Context from 'koa' for all route handler ctx parameters
- string | undefined casts for ctx.query values (ParsedUrlQuery)
- inline body type annotation in /register handler
- explicit parameter types on executeGraphQL to avoid TS2589
