ci(test): report test coverage to Coveralls (#40)

* ci(test): write coverage data to named files

Direct pytest-cov output to reports/.coverage.unit and
reports/.coverage.integration so the data files can be
combined in a subsequent step.

* ci(test): add coveralls.io coverage reporting

Combine unit and integration coverage data files into a single
LCOV report and upload to coveralls.io using coverallsapp/github-action.

* fix(ci): use COVERAGE_FILE env var instead of --cov-data-file

--cov-data-file is a coverage.py flag not recognised by pytest-cov;
COVERAGE_FILE is the correct mechanism to control the data file path.

* docs(ci): add status badge for test coverage

* chore(docs): update alt text for CI build status badge

* chore(build): ignore output from pytest code coverage
