* fix: include upstream in forwarder error messages

Updated the `forwardQuery` method to return the upstream address used
for the DNS exchange. This allows `resolveUpstream` to include the
specific upstream identity in error messages when receiving a
non-success **Rcode**.

- Modified `forwardQuery` signature to return the upstream string.
- Improved error context by identifying which upstream resolver failed.
- Enhanced observability for troubleshooting multi-upstream setups.

Enhances the logger to automatically capture and record stack traces
from errors that support verbose formatting. This provides better
context for troubleshooting failures in production by separating the
standard error message from its detailed trace.

*   Enables structured logging of stack traces for compatible errors.
*   Integrates the attribute replacer into the default `slog` handler.

* Update internal/logging/slog_helpers.go
