fix: address re-review findings for bot-presenter-handler

- Add null guard to appendToRoomMetadataArray matching removeFromRoomMetadataArray
- Remove dead if (!permissions) guard in comms-scene-handler
- Fix get-stream-info-handler to use room_id instead of place_id for room name resolution
- Replace world_name! non-null assertion with safe fallback
- Add maxLength: 256 to all cast schema string fields
- Expand promote/demote identity validation to accept Cast 2.0 streamer identities
- Add IP-based rate limiter to /cast/presentation-bot-token and /cast/streamer-token
- Add test coverage for getStreamInfoHandler, local preview bypass, room_id null guards,
  identity validation, and rate limiter
